Registration Requirements

Age: Sailors must be between 11 and 16 by date of camp

Step 1: Download and complete the following four forms:

Medical & Immunization Form (Download Here) OR Physician's Medical Examination & Immunizations

• Emergency Contact & Treatment Consent Form (Download Here)

• Liability Waiver (Download Here)

• Medication Form (Download Here) (If camper will be taking medications at the camp)

Step 2: Scan and save the above forms as a single .pdf file with the filename Lastname_Firstname.pdf

Step 3: Submit those forms as a single file by uploading here:

Note: the medical forms must be reviewed by a registered nurse (which takes some time); please submit the forms at least two weeks before the start of camp.

Registration is complete upon receipt of all required documents and payment (we cannot admit campers whose health forms have not been verified).

Cancellation Policy:

• Registrations cancelled up to 10 days before the start of camp will be refunded minus a $50 processing fee.

• Registrations cancelled after 10 days before the start of camp will not receive a refund.

The Charles River Sailing Academy is open to any and all entrants, limited only by number, age, grade level, and/or gender.
